Request a Fit Note
A fit note, sometimes called a sick note, is a document from a healthcare professional that provides advice about how your health may affect your ability to work.
If you are off work for 7 days or less
You do not usually need a fit note if you have been off work for 7 days or less.
Your employer may ask you to complete a self-certification form instead. Your employer should provide this form, or you can download one from the gov.uk website.
If you are off work for more than 7 days
If you have been off work for more than 7 days in a row, you will usually need to give your employer a fit note.
The 7 days include non-working days, such as weekends and bank holidays.
Fit notes can be issued by a GP or hospital doctor, as well as certain other healthcare professionals.
